      <title>The Experience of Mothers Caring for Their Children with Hemophilia</title>

      <description>title: The Experience of Mothers Caring for Their Children with Hemophilia abstract: 本研究採現象學研究法，深入探討母親照顧血友病病童的經驗感受。研究者於門診主 動與前來就診的病童母親接觸，在建立信任關係後取 得病童母親同意，於不受干擾 的情境下和病童母親進行深度訪談，根 據訪談指引讓病童母親敘述其照顧病童的經 驗感受；共計訪談了八位 病童母親，所得訪談的內容以錄音記錄，將錄音訪談資料 轉錄成文本 ，後依Giorgi現象學研究法進行資料的分析。研究結果發現，血友病 病 童母親的照顧經驗可歸納出五個現象主題：(1)母性角色的創傷；( 2)孤獨的承受；( 3)不確定性的焦慮；(4)維持自我的穩定；(5)望子 「成人」。本研究呈現出母親照 顧血友病病童的真實經驗及其內心感 受，可協助護理人員提供個別性的護理照顧， 促進本土化護理臨床實 務，並作為未來發展血友病整體性健康照護之依據。 The purpose of the phenomenological study is to explore the experiences of mothers who were taking care of their hemophiliac children. The informants were contacted at the outpatient department. After obtaining informants’ trust and consent, in-depth interviews were conducted to collect data with tape-recording in an interruption-free environment . A total of eight informants were interviewed. The content of the interviews was transcribed into the narrative form and analyzed using Giorgi phenomenological method. The findings of this study showed that the mothers’ experiences in taking care of their hemophiliac children were integrated into five phenomenological themes: (1) being hurtful to the maternal role; (2) being a solitary sufferer; (3) having anxiety due to the uncertainty; (4) learning to maintain an intact ego; (5) expecting for children’sgrowing-up. The findings of this study could provide nursing guidance for individual care, which may facilitate the development of culture-sensitive nursing interventions, and contribute to the development of comprehensive medical care for children with hemophilia.

      <title>Factors Related to Meaning of Life in Taiwanese Women Treated with in Vitro Fertilization</title>

      <description>title: Factors Related to Meaning of Life in Taiwanese Women Treated with in Vitro Fertilization abstract: Background/Purpose: The meaning of life can be defined as a sense of a clear aim in life and a belief that one's daily activities are meaningful. Pregnancy is clearly an important aim of women who undergo in vitro fertilization (IVF) treatment. The purpose of this study was to evaluate the meaning of life and its related factors among women who underwent IVF treatment throughout the first treatment cycle until either pregnancy was achieved or when the attempt was abandoned. Methods: This cross-sectional study was performed in a single medical center in Taiwan. A total of 149 subjects were recruited from women receiving IVF (n = 69 ) and women who had experienced IVF failure within the previous I year (n = 80). These women were classified into four subgroups according to their treatment stages: beginning of first IVF (n = 39); pregnancy/delivery (n = 22) ; continuing treatment (n = 64); and discontinuing treatment (n = 24). The Purpose in Life (PIL) test, a previously developed instrument designed to measure meaning of life, was administered to all patients at their follow- up IVF visit. Results: The mean PIL score was 99.1 +/- 19.5, which indicated that all subjects had some degree of uncertainty regarding the meaning of life; however, no significant difference in PIL score was found among the four groups. Four factors were extracted from PIL by factor analysis, among which &amp;quot;existential frustration&amp;quot; (factor 4) was highest in the continuing group and those with a lower level of education; whereas &amp;quot; being in control&amp;quot; (factor 2) was lowest in women whose infertility had a female etiology. Conclusion : Treatment stage, educational level, and etiology of infertility were found to be factors influencing the meaning of life in women undergoing IVF.

      <title>Transforming Hope: The Lived Experience of Infertile Women Who Terminated Treatment after in Vitro Fertilization Failure</title>

      <description>title: Transforming Hope: The Lived Experience of Infertile Women Who Terminated Treatment after in Vitro Fertilization Failure abstract: 生殖科技帶給不孕者高度的懷孕希望。但是，並非每個人都能實現。本研究目的為探 討經歷體外受精治療失敗後並放棄治療的不孕婦女之生活經驗。本研究採質性研究設 計，研究情境為台灣北部一所醫學中心。研究對象為曾接受體外受精治療失敗後一年 ，並已放棄治療的不孕婦女，經其同意，逐一收集，共24人。研究者依據會談指引， 採訪談法收集資料，以文字紀錄，經由Benner現象學之解釋性研究的三個策略分析： 主題分析、範例分析及典範案例分析。本研究經過該醫學中心倫理委員會審查通過， 並獲所有研究對象之同意參與。研究結果：由資料歸類出研究對象之生活經驗的主題 為轉換希望；包括三個類目：對不孕事實的接受、對高科技醫療限制的感慨、與對未 來生活的重新定位。結論：放棄治療之不孕婦女的醫療諮詢需包括告知治療的完整訊 息、持續評估治療的反應及協助患者重新自我定位。Assisted reproductive treatments provide the hope of pregnancy for infertile women, but do not always turn this hope into reality. The purpose of this study was to explore the lived experience of infertile women who terminated treatment after in vitro fertilization (IVF) failure. Using a qualitative research design, 24 subjects were recruited who had experienced IVF failure and decided to terminate their treatment. Data were collected through interviews, and analyzed using interpretive research strategies of phenomenology. This study protocol was reviewed and approved by the University Review Board for Research. Informed consent was obtained from each subject. The theme of lived experience which emerged from the data was &amp;quot;transforming hope&amp;quot;. This theme included three categories: (1) accepting the reality of infertility, (2) acknowledging the limitations of treatment involving high technology, and (3) re-identifying one's future. The results illustrated that counseling for these women should involve the provision of both positive and negative information, evaluation of the response to treatment, and assistance in defining their future.
